*This form is only required if a parent/guardian/caregiver does NOT want their child's photo to be taken for any purposes by local media partners, or District employees including the child's teacher. This only needs to be filled out once a will carry over each year. If in the future you wish to make changes in the future, please submit a signed note to your child's school requesting the Do Not Photo Request to be removed.
