Please note that all students who wish to participate in athletic programs for the school year must have an adequate physical examination.  The physical exam must be after May 1st, 2010 for the 2010/11 School Year. You must use the   DIAA Athletic Physical Form (both sides completed by doctor) and the Cape Henlopen School District Parent-Student Consent Form as well.
